IP Address - Internet Provider Capture, Display, Send.

I have searched the internet far and wide and have still failed to locate any programs/scripts that can capture an IP Address along with who the internet provider is of that IP Address.

Here is my problem......
I am a web site owner/webmaster who has a web forum. On my forum I have users sign up and then ignore my posting rules/terms of use. They post pornographic content, links, etc... My forum is set up to collect the users IP address and search IP Address, but only if they post in my forum.

What I Need:
I need a program, script, or a form that my users fill in with their information before signing up to become a member of my forum, but also that will capture the users IP Address and Internet Provider then display this on the form for the user to see. Then when the user clicks the "Agree to Terms of Use" button their information is then sent to a file on my server and or e-mails me the results. The user after clicking on the button will then be sent to the forum sign-up page.

By obtaining this information I can then hunt the forum spammers down and bring them to justice. That and get them in hot water with their Internet Provider.
