Easy, easy .....
Not for me.
save the result in a database it's not a problem but change the color
of the stars it's difficult.
Well, to be more precise, change the color of 1 star that's ok but let
the 3 first stars in red when you are on the fourth, wow ! I have
absolutly no idea how to do that !
You probably won't find a script - such a thing is quite easy to do.
You just need to put the options on the page then save the results, most
probably in a database.

One part of the problem is easy, indeed - save the results in the
database and display
them as such later, to future visitors.
The other part, considering transition effects (like when you move
your mouse over a star)
is something you should ask at comp.lang.javascript
