Storing and managing sessions in a mysql db

Can someone point me at a really solid description of how this is
done, pitfalls, etc? It seemed fairly straightforward to me when I
looked at the explanation in Lerdorf's & Tatroe's 'Programming PHP',
but now I seem to be having some problems that don't make sense.
