Namespaces and Custome DTD's

I have written a DTD for and extension of XHTML; It works fine, except
that it does not, in fact, support the extension. The page I need (as
an example) is:
http://www.ubaccess.com/david/ex6.xhtml
As referenced in the page, the DTD is at
http://www.ubaccess.com/david/base-aaa-2.dtd
The problem is, it simply doesn't add in the attributes that I wanted
it to; I'm unclear as to what is broken. Any help would be greatly
appreciated,
