Creating an Installer for a C# Application

Hey All,
I have an application that I have been using clickonce for up to this point. The issue is that clickonce is limiting the use for this application in areas. One of the areas has to do with manifest signing, and the fact that clickonce has severe limitations when NAT issues occur. It doesn't seem to be able to resolve network names well.

My question is, I want to create an installer such that someone goes to a website, and either downloads an installer (msi), or some sort of bootstrap. The bootstrap could then run, and get what it needs off the server and then do an actual install on the system. It would place it into C:/ProgramFiles etc.

What type of project is needed to create an installer, and is there any way to do a bootstrap approach?
