object reference not set to an instance of an object

Im trying to add record into a database i.e storing into an Access database using vb.net 2005 but it always comes up with an error that says "Null reference exception was unhandled---object reference not set to an instance of an object" when i tried to to catch the error -- it wont still save into my database.
it always put the error on line 5 in the code below
This is my code below ---

This error most likely means that your dataset is null. Try checking the code that you apply the datasource to your dataset and make sure that the dataset has data using the debugger.
