InvalidComObjectException / COM object ... separated from its underlyingRCW

Hi,

I am persistently getting two of these when exiting the application that
hosts my add-in. I have narrowed the problem down to two forms that are
invoked via the CommandBarButtons I place on the host application's
toolbar. I've tried everything I can think of to properly dispose of
these forms, but just can't seem to hit upon the right solution. The
problem only occurs if the forms are actually used.

Can anyone out there point me in the right direction?

When I look at the stack trace provided by the exception assistant I
see:" at
System.Windows.Forms.UnsafeNativeMethods.IMsoCompo nentManager.FRevokeComponent(Int32
dwComponentID)"

What additional things can I do to get to diagnose the root cause of
this and fix it?
