Adding a new record to both the middle tier & the back end DB

Hi folks

I am having trouble inserting a new record from a form that has controls
based upon the fields in a table in the back-end DB.

I am trying to create a situation where the web form communicates to the
middle tier which subsequently communicates to the back end DB using a SP
with SQL server.

Can somebody give me a few clues please.
