Display and manipulate DATA

People:

Is there a way to display a set of questions in form of survey, with
scales like radio buttons choices? These questions and choices are
retrieved from database.

I cannot use a datagrid for this purpose, right?

Also, for each question I want to pass a hidden field value attached to
question - how would I go about doing that?

Many thanks for your time - any ideas would be very helpful.
