How do I select a node that has both namespace and is prefixed?

<a:b> (and also in the default namespace!)
How exactly depends on the API you are using, but in general such
selection requires additional information about which namespcae URI "a"
prefix is bound to. In MSXML it's done via SelectionNamespaces property,
in .NET - via XmlNamespaceManager class.
Assume that I've XML as follows:
<?xml version="1.0"?>
<test xmlns:def="http://www.blah.com">
<a:b>
</a:b>
How do I select <a:b> tag? I'm using .Net and my problem is that I already
have a default namespace defined.
A code sample would be great.
