You need the SQL port to be open, typically 1433.
And when you specify the connection, you specify the IP or fully-qualified
host name of the target sql server.
Everything else is the same as if you were building a vb.net app that
connects to a local sql server.

That's not really good idea to make SQL Server accesible directly from
internet. Instead you can develop simple Web Application, which accepts
your requests, inqueries SQL server and returns results. In .NET it can be done
easily using ASP.NET.
