ClickOnce - database changes

We currently use a home grown update server to update our clients with our
application. The client has a service on a server that checks for updates
from our server and the clients will update their workstations from their
server.

Occasionally we need to made database schema changes and an update can
continue only if the changes are successful otherwise the client continues to
use the old version of the program. If the database changes fail then we roll
back to the original state.

How would this work with ClickOnce?
