Basic Help setting up fields in MySQL

I would be interested to pay someone for their time to help me set up
some simple Fields in a Table, fields such as name, address etc. I
would probably only take up 5 minutes, or so, of your time, but I
would like someone to simply walk me through this basic set-up over
