I have replication set up across four mySQL databases (4.0.12) on different
servers. Master Server A replicates to B. Then B replicates to C and D.
Servers B, C and D serve the web pages through a load balancer. The
replication fails about once a week on B, C or D... Here is the most recent
failure on D....
THE MASTER 'A' SUBMITTED THE FOLLOWING SQL +++++++++++

THE SLAVE 'B' RECEIVED THE SAME SQL IN relay-bin. THE SQL WAS ALSO POSTED OK
FROM 'B' TO SLAVE 'C'. THEREFORE SERVERS 'B' AND 'C' WERE OK....
HOWEVER SLAVE 'D' GOT THE FOLLOWING SQL from 'B'++++++++++++++++++++...

Another peculiar thing that I noticed is that for one insert on server 'A',
I see 29 entries in the relay-bin on server 'B', 232 entries in relay-bin of
server 'C' and 171 on 'D'. I cannot explain this phenomena...
Any thoughts on what could be wrong? Thanks.
