How to give meaningful Id for large field name?

Hi,
Could you please someone to advise me when field (label) has large name then how could we provide meaningful id for that field? I couldn’t get how to give meaningful id for these fields name.
Fields names:
1. Is this your first visit to this club?
2. Have you ever been to a health club?
3. How did you hear about our club?
4. Where did you see our ad?
5. What interested you most?
6. Do you live or work nearby?

Those are not field names.

Are you trying to figure out what value to use for the id attribute or the name attribute?

It looks to me like you have an XY problem.

Here are some possible values.
1) first_visit
2) been_before
3) heard_how
4) seen_where
5) interests
6) nearby

Yes. It works for me. I'am trying to figure out id attribute. Could you please advise me more about this? That means when these kind of questions have as a UI then how can we give meaningful id?

Since you haven't explained what you need to accomplish I can't advise you.
