Writng data to a text file from HTML form

I am a novice in HTML. I just downloaded Apache server on my Windows
2000 desktop.
It is running properly. I need to write a form in HTML which will save
the input data enterd by the user in a plain text file at the server
side.

Example:
1) My form will have two fields:
i) Name: myName
ii) Age: 33
2) User will enter "myName" and "33" and submit it.
3) At the server side, I need to write this submitted data in a text
format file.

Question:
1) What should I do in the HTML implementation and at the server side
?
