I actually use several different icons for my pages, depending on each

A server (if it does a good job), sends anything it can serve - if it is
*requested* by the client. It's up to the client (browsers, spiders, ...),
if the want to have a favicon.ico along with every opened document or not.
Some browsers do this, others not.

Some browsers will automatically request the icon when they visit the page.
Firefox does. IE does not. IE must have the page bookmarked (in the
favourites). It is nothing at all to do with servers.
