Hi
here is errors list, i have solved many but these few are not getting solved.
Please help me.
Line 5 column 113: character data is not allowed here.
Line 15 column 6: end tag for "HEAD" which is not finished.
Line 64 column 50: document type does not allow element "FORM" here.
Line 65 column 4: document type does not allow element "TR" here.
Line 72 column 7: end tag for "TABLE" which is not finished.

I have solved few others as well now following 3 left:

Line 64 column 39: document type does not allow element "FORM" here.
Line 65 column 4: document type does not allow element "TR" here.Line 65 column 4: document type does not allow element "TR" here.

these are not getting solved if any one help me out i will be thank full.

The form tag can't legally be in between the <table> tag and the <tr> tag even though it will still work properly.

move the form tag to a legal position on the page. Maybe just before the table tag will be OK. Same with the closing form tag, move it to just outside the closing table tag. If you don't want the form to cause a break in your display use display:inline to change it's block level behavior to inline level behavior:

That is caused by the missing <title> tag.
Your doctype is incomplete putting you into quirks mode. See the sticky at the top of this board. Use html4.01 strict. Then revalidate.
