need help with publisher form...

I am using publisher to create a web site. I know- no groans please. I have set up a form and when the submit button is used it goes to http 404 not found. Could someone look at my code and tell me please how to fix this problem?

There is a </form> much later in the code. Also- I tried changing the "action=" to action=mailto:[e-mail] but every time I upload changes the html reverts to the above code.

What action is this form taking? Where is the form posting the data too? I'm not familiar with Publisher.
