Having some bizarre issues with forms and IE (more IE fun!)

Here's the HTML

And the CSS

View it in IE and Firefox to see the difference. There's a few things.

1) Even though I specify list-style as NONE for the image listing, it still throws bullets on there, displacing the table, and seems to randomly choose other elements in the list to tack a bullet on to as well. This just seems really random and out there.

2) The form input fields display fine in FF or Opera...for some reason they don't want to follow the CSS otherwise in IE.

3) Z-index doesn't work properly in IE... in FF the images will "preview" fine and display on top of the other images (using the HoverBox model as a guide), however of course, in IE, it decides to ignore the z-index and just place the image underneath the one beneath it.

Any suggestions? I'm 100% lost on how to fix these bugs.
