Connecting Open office base table to HTMl Coding

HI All,
I have created 1 table in open office database.
2 fields: user name and password
i want to take 2 labels ,2 text boxes ...for name and password on HTML page.
Also 2 operations are there....Save record and new record...
On save button click ...record will get added in table and on new record click... blank form will be appear...
so i want coding on the click event of buttons.
also i need CONNECTION STRING CODING that will be in HTML coding.
Thats it only.
