SalesOrder Customization(where to place the Custom Form in the ONT_SALES_ORDERS Menu)

Hi,

I have problem Regarding the Sales Order Form Customization

1. Downloaded the original Sales Order For(OEXOEORD) and saved with different name say(OEXOEORD_CUSTOM)

2 moved to the AU_TOP/forms/US

3 Generated the .fmx in ONT_TOP/forms/US

4.created the form at the front-end
5.created function(parameter as STARTUP_MODE=SALES_ORDERS)
6.added this function to the Sub-menu:ONT_SALES_ORDERS

After i have opened the form at the Front-end(apps)
i have got many errors
like a)FRM-41014
b)FRM-41081
c)FRM-41053

since i have downloaded the Standard Form,i should not get any errors,
but errors are comming in my custom form...

please,someone help me with this problem
