Looking for a tool to create a function call cross ref. list\table\graph

I a looking for a tool that creates a cross reference table for all
global functions, and optionally class methods. That is, for each
function, which function(s) does it call and what function(s) is it
called by.

A visual directed graph is a 'nice to have' but a table, or even a
sorted list will do. It would be nice if I can select the scope:
within one or a subset of sourc files (incl. all source files).

I use W2K(SP4) and VS2005 Pro for C/C++ programming. (It is hard to
believe but this major IDE can't produce such a fundamental list,
something that compilers from decades ago could.).

I installed Doxygen + GraphViz, which is suppose to produce such
graphs but it didn't and w/o explanation.

Thanks
