Sorry, I don't have any UML version. I can only tell you that the
second edition of "UML Distilled" (apparently describing UML somewhere
between v1.2 and v1.3) has the representation you're looking for.
It's just like a class diagram with a template argument (or arguments)
name in an overlaid rectangle with dashed borders, placed above the
top right corner of the class rectangle. Something like

And, no, I don't know what UML-aware modeling package has the ability
to use that diagram. Have you tried asking in comp.object? You know,
UML is not really part of C++ language, which makes your question, uh,
