Have you checked up Rational Rose? IIRC, it does what you want.

Try doxygen: www.doxygen.org
In combination with dot this tool not only produces browsable documentation
but also nice class and collaboration diagrams.

Also many UML tools have a reverse engineering feature, though often you
spend quite some time to clean up the clutter.
