Adjust RGB pixels using a known set of values in image

I want to place a special header in a BMP file that has pure black then white then red then green then blue values. I will print the image, scan it back in and want to adjust the entire image using the reference colors. The pure Black White Red Green and Blue values may have shifted because of the printer and/or scanner, but I can adjust each pixel using the references in the image as the entire image would change.

My need is to scan the image back in and have the same color values as when it was created in code.
Any help would be appreciated...
