Help with path error

I'M HAVING THIS PROBLEM WITH MY WEBSITE, I DEPLOYED IT ON TO TE SERVER
AND ON THE SERVER IT WORKS WELL BUT WHEN i TRY TO INSERT DATA WHEN ON
CLIENT I GET THIS ERROR:
