I use Axialis. http://www.axialis.com/ to design icons and create ico
files.

Others have their favorites, too.

The Icon Book by William Horton (ISBN: 0471599018) is one of my
favorite books of all time. It explains the whole theory around icons
and introduces the concept of an "iconic language" whereby you can
create a family of icons that have consistency throughout.

Check this out, it's also pretty cool, and best of all, it's free download.
