I'm not sure but I think this happen when this file is already saved
in the cache (when you open this file again)

When you choose "open" instead of "save", IE will save the file in
"Temporary Internet Files" and use associated program to open it. To avoid
duplicate file name in the same temp directory, the surfix is always
appended (starting from index 1). If you choose "save", the file name
suggested in "Content-Disposition" header is used as default file name.
