Assuming you are using Visual Studio...
To add the report itself:
Project->Add New Item, then select Crystal Report
To add the viewer, add an instance of the Crystal Report Viewer from the
toolbox.
Setting viewer properties and report design is covered in numerous articles
in the MSDN help. Search using "Crystal Reports".
