Online form must match hardcopy

Hello, all. We have a form here:
http://www.uncw.edu/stuaff/SDS/images/imunform.PDF that we need to re-create
in an ASP.NET app. One problem: the client wants it to look identical to
this PDF file. Is it possible to create an ASP.NET form that will match this
form or are we just dreaming? Thank you.

You should be able to get it pretty close. Keep in mind you won't ever get
identical. You'll probably start with problems with Page Breaks in the HTML,
for example.

Have you considered a web-based report writer, like Crystal or MS Reporting
Services? They can output in PDF format.

We have Visual Studio 2003 Pro. Does it include Crystal Reports? Thanks.

No. But if you have a SQL Server license, Reporting Services is free
