IIS Crash - Out-of-process ISAPI extension request failed

Hi,
We are running ASP application in our Production
environment on "Windows 2000 Advanced Server" SP4 , IIS
5.0.

Occationally We see a IIS crash,
with the following info on Event Log

Event Type: Warning
Event Source: W3SVC
Event Category: None
Event ID: 37
Date: 12/5/2003
Time: 6:51:22 AM
User: N/A
Computer: LABOPS3
Description:
Out of process application '/LM/W3SVC/1/Root/XXXX'
terminated unexpectedly.
For additional information specific to this message please
visit the Microsoft Online Support site located at:
http://www.microsoft.com/contentredirect.asp.

When I checked the IIS log,
I see following info at the same timestamp with "Out-of-
process ISAPI extension request failed" (in the second
request).

Ofcourse, the above time is PST(GMT -8:00) and the below
is GMT.

GET /XXXX/IJKL.asp
UserSessionId=706B6FD0A5434AAC9A4B324741D0712F&Sch edulerId=
6945 200 Mozilla/4.0+(compatible;+MSIE+6.0;+Windows+NT+5.0)

2003-12-05 14:51:22 A.B.C.D - W.X.Y.Z 443
GET /XXXX/IJKL.asp
UserSessionId=706B6FD0A5434AAC9A4B324741D0712F&Sch edulerId=
6945%20PMOut-of-process+ISAPI+extension+request+failed.
500 Mozilla/4.0+(compatible;+MSIE+6.0;+Windows+NT+5.0)
Can some one help me in solving the riddle.
I have visted all possible site, but no useful info I
could find.
One of the MS support says, this problem is fixed in SP4.
We have installed SP4, and uptodate with all the Critical
Patches.
