Content manged forms

Hi,

Wonder if anyone can suggest anything for this.

My customer has a market research site, and wants to publish regular
questionaires on it as a web form and store the data in an access db.
Simple enough...

But he wants to be able to create the forms through a back end. i.e.
create a new research campaign and define what fields are within the
questionaire, are they text inputs, tick boxes, how are they names etc
then save all that data. And do it all through an easy web interface.

Has anyone got any suggestions on the best way to aproach this? Or can
someone point me in the right direction?

Hope it makes sense, and any help is apreciated.
