Invoice Report: Inputting contact details

I reworked my database to place all of the contacts in one table called the "Clients" Table (I was using the approach described by Allen Browne's model http://allenbrowne.com/AppHuman.html). I am experiencing a problem with an invoice report, where I tried different approaches to include the insurer contact details but the information appears blank (I tried to do a dlookup in the report, perform a query but all of the approaches showed up blank). Is there a way to be perform a query with this approach?

So I added some fields to the Invoice table for the Insurer name & address so that this information is referenced in the invoice report. But I guess I will be breaking the rule of relational databases, where I will be duplicating information in more than one table.
