filtering combobox and adding new records

Greetings,

Maybe this question has been asked before, I have a form that got couple of combobox.The first combobox must filter second, second to third so on, the form is based on a query from these tables(tbldesignation, tblestablishment and tblstructure).

The other issue I have I can't add new record, the add new record in navigation toolbar is disable on the form properties I enable (addition,edit and delete)but still it's not working.

Anyone pls assist.
