Import a Form and Data to Microsoft Access

I need to fill in a pdf form, which I want to export to Microsoft Access.I prepared an Excel spreadsheet with the data needed to populate the form in Access.I intend to export the spreadsheet to the same Access file. My question is
  1. How can I import the pdf form without losing the visual features of the subject form?
  2. If I export the spreadsheet to the same Access file, how do I get the subject data imported from the spreadsheet,to be mapped in the form, to populate the required datafields of the form.
  3. I would need multiple forms with unique data combinations extracted from the imported data file in the same Access file?
  4. Can any of the members assist with info to create and establish the documents to achieve the mentioned outcomes? I am looking forward to your replies and thank you in advance for the attention herein and hereto. Kind regards,

1. Create a form in Access that mimics the look of the PDF.

2. You map the fields in the table to the controls in the form.

3. I have no idea what you said here. Please create a new thread for it with a better explanation.
