Access Query filter criteria self duplicating

Hi,
I have specified form textbox data as query filter criteria,and since then, every time I run the query, the criteria are self duplicating on random lines below the original. Also, there seems to be additional coulmns being created on the right end of the query... I'm submitting a screen shot for clarity. Any ideas?

Please be aware that most of us can not see your attached image because our company IT tends to block such sites.

Therefore, it is always much better to put your information within the post, not rely on a picture unless specifically asked by one of us.
IN this case, post the SQL too.

The other issue with your image is that you've redacted several of your fields... we can't properly evaluate your query without that information.

When you say "Run the query"
Do mean from the UI or from VBA or Macro?

Access often reinterprets your SQL if it feels it can/needs to.

Before we can comment more specifically we would need to see (in SQL form) what it was you originally entered and what was saved by Access.
