What type of query should I be doing?

Hi All,

Attached is a small database. Please look at the spread sheet first.

There are 3 worksheets. First one is the raw data. In the second spread sheet I adjusted it into a format to pull the data into Access table. The third spreadsheet is the output required. How do I do this as obviously I have to do a union query.

I will not be using the spread sheet. I will be inputting the data into the access table and would like to get the output I want as shown in the the third spreadsheet.
