How to populate multiple fields on my form by using a combo box?

How do I populate multiple fields on a form by using a combo box. My combo box has 3 col. When I make a selection from my combo box all 3 fields should be filled in on my form.
eg. Form Fields: Name, D O B, U C I.
combo box: Col 1 name, col 2 D O B, col 3 UCI.

Each row contains Name DOB UCI. when I select a name, my form should be populated by my selection.
eg, Tom Andy 12/23/45 2341567

no one is going to write all that code out but im sure google search for java form filler might pull something useful.

you really did not say much as far as answering the question. This is what I mean: I am filling in a form field from a combo box and - at the same time - want to update a 2nd and 3rd field in the form with other data located in the drop down record.
