Numerical Keypad form wanted

Has anybody got a numerical key pad form , or where can one download one, that i can use inside another form? It must have 0-9,+ - X / =
I want to be able to click on figures and get a total.
I dont need the microsoft office version that pops up, but would like to incorporate it inside a form for a business that can just click on the numerical buttons.
