Microsoft Access Data Collection Tool

I am trying to use the Microsoft collection tool to (1) collect data from individuals and (2) once they have submitted their information to have them validate or update as required. I have created the database fields and under External Data / Create E-mail created an e-mail. However, the form shows up backwards...(see attachment). Someone else tried it and it worked correctly for them...is there any way to fix this?

Somewhere you've switched a setting "Left to Right" or "Right to Left" though I have no idea where exactly you have managed to do it.
