Open specific or new record

So I am trying to make a database that has a bunch of tables each with a common reference of PATID.

Table1
PATID
Contact details etc

Table2
Key
PATID
Questionaire answers

Want the form for table 1 to open form for table 2 on existing record if there is one or new record if not.

Table 1 has a one-to-one relationship with table 2. On;y split the tables as too much info to put on one table.

If my guess to understand your question is correct, are you looking for a subform then contains record information in table 2, base on the same PATID in table 1? or is it you want those fields to be display within the same form that shows table 1's records?

you can do that on the record source within the field's property, saying selecting the field's name for table 1's PATID = table 2's PATID
