multiple identical table in microsoft access

Hi all,

I have a project called bariatic surgery effect on weight. And I have 5 identical tables
1.BariaticEffectOnWeight_baseline
2.BariaticEffectOnWeight_6months
3.BariaticEffectOnWeight_12months
4.BariaticEffectOnWeight_36months
5.BariaticEffectOnWeight_60months
and I have the fields;
1. Date
2. study No.
3. MRN
4. Date of Surgery
5. Sex
6. Date of Birth
7. Type of Surgery
8. Indication of Surgery

my question is; the data that have entered from these 8 fields should appear from 6 months-60 months. I mean when the data entry entered data from baseline it will be part also of 6 months - 60 months as they the same table. however to make easy in the part of the data entry he will then start key in after the field of Indication of Surgery. Is it part of relationship? Or Am I wrong with field properties? I am newbie here and I will be very glad if you could help on this...I attached for reference..

by the way I am doing this project in Microsoft access 2003.

In addition from 6 months up to 60 months they will be part of those 8 fields mentioned. thanks. I will be waiting for a quick response.

I forgot to mention that I have the first field for each table called Sn_No set as autonumber and its my primary key. so it would be 9 fields from the baseline to be shown also through the rest of the table.
