advanced search in ms access form

hi,
I need the code which can search in selected fields and can be control with "AND" and "OR" options, ie:
1- we type some kewords
2- we select "AND" or "OR" option
3- we click on the search button
4- a list of records with selected words appears
5- with click on each item we go to selected record


I build a database of diseases in each record there is an ID, Title, Discription, and Symptoms fields.

indeed I'm a MD Doctor, so please help me with details
