Need to load dates in 2 fields in same record depending on Togglebutton or check box

I have to log Regular services that occur as required but not more than
31 days later than the previous. Additionally a major service must occur
within 6 months of the previous major service. A major service also
includes all the items in the regular service. Sample data below

I want to have the form load the dates of the Regular Services every time
but the major service only when it is performed. (The 2 digit and 3 digit
numbers are the elapsed day between services)

Eg. 01/07/07, 28/12/07, 25/06/08, 17/12/08 the Major service also covers
the Regular service.

I have been thinking of a toggle or a check box for the major, but I
can't figure out how to get that in to 2 fields in the same record.
