Data entry form leave empty and remain 4 row

Hi,

I am new in Access database. How am i going to remain 4 row for data entry as shown in my attachment?Please reply as soon as possible

anyway, if i using own creation text not generate by access, how to insert data to database.

You can replace the subform with simply a list control with the SQL statement to display those very fields. that would get rid of the forth row.
