Table/Subform Issues

Hospital project: I'm new AccessDB design - trying to created a DB in MS Access 2000 which keeps track of internal incidents involving our staff and patients. I have created 4 tables; TBL_Incident_Record, TBL_Patient, TBL_Staff and TBL_Incident Code. I need to do monthly reports based on the Incident code, the staff member, and the patient (seperately).

After creating the tables attempting to set up the relationship, my 1:M get reversed. Are my relationships correct? Do I need to use subforms for adding Incident Records which can contain many Patients and many Staffs on each record – always only one Incident Code.


TBL_Incident_Record – PK – Incident ID
FK – Patient ID
FK – StaffID
FK – IncidentCodeID

TBL_Patient – PK – PatientID

TBL_Staff – PK – StaffID

TBL_IncidentCode – PK - IncidentCodeID

Relationships:
TBL_Incident_Record (1: M) with both TBL_Patient and TBL_Staff
TBL_Incident_Record(1:1) with TBL_IncidentCode
