Spwanable Forms?

Is there some way that I can make a form that is spawnable?

Example:
User asked a question. How many Dogs? (some number)
Then
Dog (first number)
Info about the dog.

Dog (Second number)
Info

...

Dog (Last Number)
Info

I thought about implementing it with some sort of subform with a loop but I am unsure of how to do this. Can anyone help?

Rather than start with thinking about forms, the first thing you should do is design your table structure. Queries come next, then forms, and lastly reports.

I have tables and everything else seems to be working great. But what I want to do is add another table with this information and relate it back to the original.

I'm not sure I follow on what you mean by spawning a form. The phrasing sounds like you're trying to create and open multiple forms but that doesn't seem right.

Rather it sounds more like you're trying to spawn a user defined object much like a game would spawn monsters for a battle.

Since you already have the tables, let's have a look at their structure.

Here is an example of how to post table MetaData :
Table Name=tblStudent
Expand|Select|Wrap|Line Numbers
  1. Field; Type; IndexInfo
  2.       StudentID; AutoNumber; PK
  3.       Family; String; FK
  4.       Name; String
  5.       University; String; FK
  6.       Mark; Numeric
  7.       LastAttendance; Date/Time
