All textboxes show "#Name?"

I have created a form that uses linked combo boxes to filter the
results. When the filter runs, all of the text boxes in the form
become populated with "#Name?". The record counter at the bottom
indicates that the correct number of records were filtered, but I can
not see any of the data.

Why and how do I fix this?

So either (a) the recordsource is incorrect, or (b) the controlsource
is incorrect.
