Access-Open a form from the input of 2 textboxes and an option group.

Hi, I have a form in Access2007 with two text boxes(date text boxes),1 option group with 3 radio buttons, and a command button. I would like that when the user types the 2 dates and selects one of the options a report is created with that info. I have the report from where the info can be pull. Do you have an example code for this situation, I need it badly.

You don't say what you want the option to do. However, I'm going to assume it is a different report each time.
